Submission of Application

All applications and supporting documents must be received no later than Sept. 1, 2026. Please use the P.O. Box mailing address ONLY for submitting tax returns.

All of the information that I have provided in this application and in the enclosed letters is true and complete, to the best of my knowledge. I certify that I am currently enrolled and in good standing as a medical student for the 2026-2027 academic year, and that I am a resident or attending school in the Oakland-San Francisco Bay Area, and am eligible to receive scholarships granted under this program. I hereby authorize Sinkler Miller Medical Association to use any information contained in the application for the purpose of promoting and publishing the program, or as legally required or permitted by law.

All award recipients agree to submit the following by Sept. 1, 2026:
  1. A letter of acknowledgement to Sinkler Miller Medical Association, stating what the scholarship means in their lives.
  2. A high resolution photograph that may be used for the purpose of promoting the program either at our live event or in social media as legally required or permitted by law prior to receipt of the award.
To comply with the provisions of the Family Educational Rights and Privacy Act of 1974, permission is hereby given to applicant school officials to release the applicants secondary and tertiary school records and other requested information for consideration in the program.
